Are you bench bleeding the slaves before installing them?

Do you have the TOB in the fork the correct way? the fork shouldn't have much play

What do you mean 'slave went out'? Did the push rod come out of the cylinder? Did the seal blow out?

Do you have the right slave cylinder?

Also, i belive the word you cant think of is 'and'.

The seal blew out.  And I  did double check  to make sure  I have the right  one.

Try a different brand. I've heard non-oem slaves are hit or miss

TOB is on correctly. Not sure what else to do so I ges  I'll just put it back together n try an oem  slave.
